ULTRA FOOTBALL PITCH HIRE CONSENT (Please read carefully before proceeding)

This agreement is between Ultra Football and me and will be deemed to have commenced upon acceptance by Ultra Football of my application to allow my child and all guests invited by me or my child to participate in activities during the Ultra Football Pitch Hire.

In consideration of and as a condition of acceptance for the activities on the day and date confirmed by Ultra Football, I acknowledge and agree with the following:

1. In this agreement

"Claim" means and includes any action, suit, proceeding, claim, demand, damage, cost or expense however arising including but not limited to negligence.

"Activities" mean any or all activities which take place at the Ultra Football Experience.

2. I acknowledge that whilst the participant participates in activities, exposure to certain risk may occur, these include but are not limited to the following:

- Serious injuries

- Other participants may act in an international and dangerous manner or with lack of skill; and/or

- Property may be damaged, lost or destroyed.

I acknowledges that accidents can and often do happen during activities which may result in the participant being injured or killed, or my property being damaged.

GUIDELINES FOR PARTICIPATION

I understand and confirm that I have explained to parents/guardians of participants invited by me or my child that:

- The person attending an Ultra Football Experience where the activities will occur, will engage in physical activities where risk may be present.

- Parents/guardians have been advised of this consent form and potential risks during activities and provided permission for their child to participate in any of the activities.

- Supervisors and accompanying adults need to ensure that the group of participants maintain order and follow the rules during all activities.

I agree to:

- In the case of injury, inform the activities co-ordinator immediately.

- Make known to the activities co-ordinator if the participant have any existing medical conditions or injuries so that Ultra Football staff can look after them properly in the case of an emergency;

- Not allow the participant to participate in the activities if they are affected by medication/drugs;

- For the participation safety, explain to them that they are not to touch, climb or use any unauthorised equipment.

- Compensate Ultra Football for the replacement value for any intentional and malicious damage to the Ultra Football property caused by guests of the Ultra Football experience.

FITNESS TO PARTICIPATE

- I warrant and declare that the participant is medically and physically fit and able to participate in the activities;

- I will immediately notify Ultra Football of any chance to any participant's medical condition, fitness or ability to participate after the date of this consent form and prior to the Experience commencing.

- I understand that if Ultra Football is not provided with all the relevant and necessary information about the participant's health and capacity to participate in activities, they will not be able to appreciate the risk of harm or injury to the participant in providing instruction and in allowing the participant to participate in the activities;

- I understand that Ultra Football will continue to rely upon this consent form as evidence of all participant's fitness and ability to participate unless notified otherwise;

PARTICIPANT BEHAVIOUR

I understand that in the event of any guests invited by me misbehave or engage in behaviour posing a danger to himself/herself or others during the activities or act in a manner that increases the risk of damage to Ultra Football property, the event will be paused until such time as the guests are deemed to have returned to acceptable behaviour as determined by Ultra Football management. Failure to return to acceptable behaviour may result in the experience being terminated by Ultra Football Management.

I have explained the expectations and rules to all participants, and the participants are aware of Ultra Football's expectations during all the activities. I have read, understood, acknowledged and agree to the above terms, including the warning related to participant behaviour.
